Preapplication Appointment Form

To submit your application: 

• Fill in all applicable sections.
• Come to City Hall during Counter Hours to submit your application, attachments, and payment
  OR
• Mail your application and attachments, with payment by mail or phone. (To pay by credit card, call 503-786-7555 during Counter Hours.)
• When we receive your complete application, we will schedule your appointment per the timelines on the form.
